ABSTRACT
Patients with COPD run high risk of OSA whose identification and diagnosis is clinical challenge. STOP BANG questionnaire might overdiagnose OSA in COPD patients who are old, male and comorbid with hypertension, underdiagnose OSA in COPD patients with low BMI and small neck circumference. Respiratory polypgraph might overdiagnose OSA in COPD patients with low baseline PaO2, underdiagnose OSA in COPD patients comorbid with insomnia. Even PSG without ET_CO2 channel might misdiagnose COPD induced nocturnal alveolar hypo-ventilation as OSA. Modification of STOP BANG cut-off point, clinical examination to evaluate oropharyngeal size using Mallampatti score, measurement of awake PaCO2, PSG with ET_CO2 channel are possible solutions to identify and diagnose OSA in COPD.
RÉSUMÉ
Les patients atteints de BPCO courent un risque élevé de SAOS dont l'identification et le diagnostic constituent un défi clinique. Le questionnaire STOP BANG pourrait surdiagnostiquer l'AOS chez les patients BPCO âgés, de sexe masculin et comorbides avec l'hypertension, sous-diagnostiquer l'AOS chez les patients BPCO avec un IMC bas et une petite circonférence du cou. Le polygraphe respiratoire pourrait surdiagnostiquer l'AOS chez les patients atteints de BPCO avec une faible PaO2 de base, sous-diagnostiquer l'AOS chez les patients atteints de BPCO comorbide avec l'insomnie. Même la PSG sans canal ET_CO2 pourrait mal diagnostiquer l'hypoventilation alvéolaire nocturne induite par la BPCO comme OSA. Modification du seuil STOP BANG, examen clinique pour évaluer la taille oropharyngée à l'aide du score de Mallampatti, mesure de la PaCO2 éveillée, PSG avec canal ET_CO2 sont des solutions possibles pour identifier et diagnostiquer le SAOS dans la BPCO.
